PURPOSE：
Applicant made the arrangements for the travel or tour in Japan.
If applicant request a multiple-entry visa.

  • All documents must be in A4 size.
    Documents of other size must be copied into size A4 and please submit together with the original..
  • Photo size is 45×35mm(2×1.4in).
    Application form and photo size were revised.　Please use revised application form and photo(45×35ｍｍ）.
  • Please DO NOT USE FRICTION ERASABLE PEN in visa application form and in all documents.
  • Please make sure all documents legally obtained and DO NOT SUBMIT FALSIFIED DOCUMENTS.
  • WE FREQUENTLY GET QUESTIONS ON "HOW MUCH"money should be indicated on the bank certificate.
    Although the embassy did not release any guidelines on this or any minimum amount. 　It is best to show your biggest but truthful balance.
    Please call for personal appearance, requirements and others.
  •  Please DO NOT PURCHASE any form of travel arrangement prior to the release of your visa application results. Delays and Denials do occur.

【REQUIREMENTS】
All documents must be original unless otherwise stated.

1: PHILIPPINE PASSPORT

  • Broken passport is not accepted.
    Passports must be signed and must have at least two(2) blank visa pages.

2: VISA APPLICATION FORM

  • Application Form Size: A-4 size(21.0×29.7cm)
    Application form should be filled out all items correctly.
    If items is not applicable, please fill in [N/A].
    Application form without date of application, signature and with blank item will not be accepted.
    For the date of application, please fill in the date of submission to RELI TOURS AND TRAVEL (or to the embassy in case of direct application).
    Embassy may deny the application that is filled out incorrectly or wrong information.
    Please do not used Friction Erasable Pen in Visa Application Form and in all documents.

3: PHOTO

  • Photo size is 45×35mm(2×1.4in) with white background.
    Photo must be taken within 6 months.
    Please write applicant's name and birthday on back side of the photo.
    Photo must be pasted on the application form. -- Please put on glue each corners

 

※ IF THERE IS /ARE A USED JAPANESE VISA ON YOUR PASSPORT.
　　THE FOLLOWING 4, 5 ARE UNNECESSARY.
　　　PLEAS ATTACH YOUR OLD PASSPORT,
　　　IF THERE IS / ARE USED JAPAN VISA (LANDING PERMISSION) ON YOUR OLD PASSPORT.

4:BIRTH CERTIFICATE -ORIGINAL-

  • It must be issued within one year from PSA Main Office/Serbilis Outlet Center(Nationwide).
    If the birth certificate from Philippine Statistics Office(PSA) is unreadable, or has incomplete information, please submit the birth certificate issued by PSA together with a birth certificate issued by the Local Civil Registrar.If the birth certification is "late registration",
    please submit in addition, a Baptismal Certificate(Original within 3 months from issuance) and School Record(form 137) from high school or elementary.(Original within 3 months from issuance)
    If there is no record in PSA,
    please submit a "Certificate of None-Record" from PSA together with a birth certificate from the Local Civil registrar.

5:MARRIAGE CERTIFICATE(if the applicant is married) -ORIGINAL-

  • It must be issued within one year from PSA Main Office/Serbilis Outlet Center(Nationwide).
    In case there is no record in PSA,
    please submit a "Certificate of None-Record" from PSA together with the Marriage certificate from the Local Civil Registrar.

AUTHORIZATION LETTER to Japan Embassy for PSA, Fill in and Attach with other required documents.
※If the applicant can submit old / valid passport with used Japan Visa, They allow to exempt submission their Birth Certificate, Marriage Certificate and Authorization Letter above.

6:DAILY SCHEDULE IN JAPAN(TAIZAI NITTEIHYO)

【If applicant will shoulder part or all of his/her travel expenses】

7:BANK CERTIFICATE -ORIGINAL-

  • Validity of Bank Certificate is three (3) months from the date of issue.
    ※Please submit your personal accounts,not company/business accounts.

8:APPLICANT'S INCOME TAX RETURN(ITR)(Form 2316) clear photocopy(latest).

  • - It must have amount of gross income and a stamp of recipient, if tax is not withheld
    (e.g. the self-employed).
    【ADDITIONAL REQUIREMENTS】
    - If (8) is not available and there is no ADB (Average Daily Balance) on (7) Bank Certificate,
    submit Bank Statement to prove transaction for the last 3 months.

【If Guarantor who lives in the Philippines will shoulder part or all of the applicant's travel expenses】

9:GUARANTEE LETTER

 

10:PROOF OF RELATIONSHIP BETWEEN APPLICANT AND GUARANTOR (e.g.Birth Certificate etc.)

 

11:BANK CERTIFICATE(original) and

  • INCOME TAX RETURN(ITR From 2316) OF GUARANTOR

 

【If Applicant is not Philippines Nationality】
12:PHOTOCOPY OF APPLICANT'S ALIEN CERTIFICATE OF REGISTRATION(ACR)

  • ISSUED BY THE PHILIPPINES GOVERNMENT.
    Foreign residents with long-term or permanent status in the Philippines applying for a visa must submit in addition to the above requirements(except birth certificate and marriage certificate)

※Visa application of foreigners who are on short-term(temporary) visa in the Philippines can not be accepted.
Please apply at the Japanese Embassy / Consulate General with jurisdiction over the area in resides or the country of which the applicant`s passport was issued.
