【CHANGED THE VALIDITY OF COE】(March 1, 2022)
◆Issued from February 1, 2022 to July 31, 2022　>> Valid for Six(6) months

 

From June 23rd, visa applicants with a Certificate of Eligibility for Residence Status who are staying with their family (Spouse or Child of Japanese National, Dependent, and Specific Activities Notification No. 53 and 54) will be required to submit a tuberculosis-free certificate after undergoing a chest X-ray, etc. The tuberculosis-free certificate will be issued by a designated medical institution.

  • All documents must be in A4 size.
    Documents of other size must be copied into size A4 and please submit together with the original..
  • Photo size is 45×35mm(2×1.4in).
    Application form and photo size were revised.　Please use revised application form and photo(45×35ｍｍ）.
  • Please DO NOT USE FRICTION ERASABLE PEN in visa application form and in all documents.
  • Please make sure all documents legally obtained and DO NOT SUBMIT FALSIFIED DOCUMENTS.
  • WE FREQUENTLY GET QUESTIONS ON "HOW MUCH"money should be indicated on the bank certificate.
    Although the embassy did not release any guidelines on this or any minimum amount. 　It is best to show your biggest but truthful balance.
    Please call for personal appearance, requirements and others.
  •  Please DO NOT PURCHASE any form of travel arrangement prior to the release of your visa application results. Delays and Denials do occur.

【REQUIREMENTS】
All documents must be original unless otherwise stated.

1: PHILIPPINE PASSPORT

  • Broken passport is not accepted.
    Passports must be signed and must have at least two(2) blank visa pages.

2: VISA APPLICATION FORM

  • Application Form Size: A-4 size(21.0×29.7cm)
    Application form should be filled in all items correctly.
    If items is not applicable, please fill in [N/A].
    　Application form without date of application, signature and with blank item will not be accepted.
    For the date of application, please fill in the date of submission to RELI TOURS AND TRAVEL (or to the embassy in case of direct application).
    Embassy may deny the application that is filled in incorrectly or wrong information.

3: PHOTO

  • Photo size is 45×35mm(2×1.4in). with white background.
    Photo must be taken within 6 months.
    Please write applicant's name and birthday on back side of the photo.
    Photo must be pasted on the application form. -- Please put on glue each corners

4: PHOTOCOPY OF CERTIFICATE OF ELIGIBILITY(COE)

  • Certificate of Eligibility (photocopy, no need original)
    * If you received a Certificate of eligibility by email from Japan Immigration, submit a photocopy of the email.
  • ※If your Status of Residence in Japan is
    Spouse or Child of Japanese National,
    Spouse or Child of Permanent Resident,
    Long Term Resident    ,
    Dependent,
    you have to submit 5 (Birth Certificate) and 6 (Marriage Certificate) below.

5:BIRTH CERTIFICATE -ORIGINAL-

  • It must be issued within one year from PSA Main Office/Serbilis Outlet Center(Nationwide).
    If the birth certificate from Philippine Statistics Office(PSA) is unreadable, or has incomplete information,
    please submit the birth certificate issued by PSA together with a birth certificate issued by the Local Civil Registrar.If the birth certification is "late registration",
    please submit in addition, a Baptismal Certificate(Both original issued within 3 months and Make Sure the contact number & address Is present in the issued document.) and School Record[form 137](Both original issued within 3 months and Make Sure the contact number & address Is present in the issued document.) from high school or elementary.For clients who will submit School Records and Baptismall certificate. Make
    Sure the contact details Is present in the issued document.
  • If there is no record in PSA, please submit a "Certificate of None-Record" from PSA together with a birth certificate from the Local Civil registrar.

6:MARRIAGE CERTIFICATE(if the applicant is married) -ORIGINAL-

  • It must be issued within one year from PSA Main Office/Serbilis Outlet Center(Nationwide).
    In case there is no record in PSA,
    please submit a "Certificate of None-Record" from PSA together with the Marriage certificate from the Local Civil Registrar.
  • For permanent residents, applications under Notifications No. 3, 5(ハ), and 6(ハ), or "Designated Activities (Notification No. 43)" are related to Japanese descendants remaining in Japan. Therefore, in addition to the required documents, a family register transcript, family tree, and old and new photographs of the first or second generation Japanese descendants are required.
  • For Designated Activities, Notification No. 1, applications are for individuals working at a foreign diplomatic mission in Japan who employ a personal staff member. Therefore, the employer's identification card (issued by the diplomatic mission where they work) and a copy of the identification card issued by the Japanese Ministry of Foreign Affairs are required. If the employer's residence status is "Diplomatic," the submission of a resident registration certificate and income certificate is waived. (Required documents may vary depending on the application; please inquire.)
