PURPOSE：A Join a package tour for sightseeing arranged by registered travel agency of the Embassy.
・For the Philippine national only.
・The term of stay in Japan must be 15 days.

1: PHILIPPINE PASSPORT
Broken passport is not accepted.
Passports must be signed and must have at least two(2) blank visa pages.
2: VISA APPLICATION FORM
Application Form Size: A-4 size(21.0×29.7cm)
Application form should be filled out all items correctly.
If items is not applicable, please fill in [N/A].
Application form without date of application, signature and with blank item will not be accepted.
For the date of application, please fill in the date of submission to RELI TOURS AND TRAVEL (or to the embassy in case of direct application).
Embassy may deny the application that is filled out incorrectly or wrong information.
Please do not used Friction Erasable Pen in Visa Application Form and in all documents.
3: PHOTO
Size: 45×35mm, with white background.
Photo must be taken within 6 months.
Please write applicant's name and birthday on back side of the photo.
Photo must be pasted on the application form. -- Please put on glue each corners
※ IF THERE IS /ARE A USED JAPANESE VISA ON YOUR PASSPORT.
THE FOLLOWING 4, 5 ARE UNNECESSARY.
PLEAS ATTACH YOUR OLD PASSPORT,
IF THERE IS / ARE USED JAPAN VISA (LANDING PERMISSION) ON YOUR OLD PASSPORT.
4:BIRTH CERTIFICATE -ORIGINAL-
It must be issued within one year from PSA Main Office/Serbilis Outlet Center(Nationwide).
If the birth certificate from Philippine Statistics Office(PSA) is unreadable, or has incomplete information, please submit the birth certificate issued by PSA together with a birth certificate issued by the Local Civil Registrar.
If the birth certification is "late registration",
please submit in addition, a Baptismal Certificate and School Record(form 137) from high school or elementary and School Yearbook(if possible).
If there is no record in PSA,
please submit a "Certificate of None-Record" from PSA together with a birth certificate from the Local Civil registrar.
5:MARRIAGE CERTIFICATE(if the applicant is married) -ORIGINAL-
It must be issued within one year from PSA Main Office/Serbilis Outlet Center(Nationwide).
In case there is no record in PSA,
please submit a "Certificate of None-Record" from PSA together with the Marriage certificate from the Local Civil Registrar.
Please attach AUTHORIZATION LETTER for each applicants*"4", "5": AUTHORIZATION LETTER to JAPAN EMBASSY for PSA
AUTHORIZATION LETTER to Japan Embassy for PSA, Fill in and Attach with other required documents.
※If the applicant can submit old / valid passport with used Japan Visa, They allow to exempt submission their Birth Certificate, Marriage Certificate and Authorization Letter above.
